How to complete a surgical report

Once the client has registered and completed their first form on the Canine Cruciate Registry you will be able to access the client account and complete your veterinary initial assessment form.

Once the veterinary initial assessment is completed you will be able to access the surgical report.

Within this form you confirm the date of the procedure, what clinic the surgery was performed at and the operating surgeon.

You then complete information about your procedure filling in as much or as little as you want.

If you perform the same type of procedure regularly you can save this form as a default making it easier to fill out surgical reports in the future.

Once you have completed the form press save.

Completing this form automatically triggers follow up forms to be sent to the owner to gain information  about the dog’s recovery and how they’re coping post-surgery. These forms also include a friends and family survey, where you can receive valuable feedback for your practice.

It is made clear to the owner that completing the forms does not replace post operative checks and that they still need to contact the practice if they have any concerns.
