Delegates can input information on your behalf and can be vital in ensuring that surgical information is added. Delegates are often veterinary nurses or members of the administration team.

Delegates can input information on your behalf, assisting in completing surgical records and inputting other information.
There are two ways to add a delegate on the Canine Cruciate Registry
- If you have not already registered then you can add a delegate during registration. On the veterinary surgeon’s registration form, you fill out your user details, practice details and any delegate information down here. You must ensure that you have their consent to provide their name, e-mail address and phone number. Once you have added the delegate, then you can submit the form and both you and the delegate will receive an e-mail with your registration details within two working days.
- If you have already registered and have an account on the Canine Cruciate Registry then you can e-mail amplitude at customer.support@amplitudeclinical.com and request that a delegate is added to your account. As before you must ensure you have their consent to provide their name e-mail address and telephone number.
